PLEASE READ THIS INFORMATION FOR CONSUMERS IN THE STATE OF TEXAS CAREFULLY AND IN ITS ENTIRETY.

House Bill 4224 (89th Regular Session), which went into effect on Sept. 1, 2025, requires licensees to, per Section 181.105 of the Health and Safety Code, prominently post on their website and at any facility detailed instructions for a consumer to:

  • Method of which consumers can request the health care records from the licensee:

    • ​All records requests must be made in writing and sent to tiffany@tlctherapyatx.com or via mail (USPS, UPS, FedEx) to the practice’s mailing address. If requesting records by mail, please notify provider by phone (512-879-9667) or via email address listed above:

      • TLC Therapy, PLLC, PO Box #76, Manchaca, TX 78652.

      • If there is a joint file, all parties MUST sign a release of information for records to be released.

      • Electronic records will be shared through the practice's secure portal.

      • Fees apply.

  • The Texas Behavioral Health Executive Council can be contacted through various modalities:
